ABSTRACT

Objective:To explore the expression of polypyrimidine tract-binding protein 1 (PTBP1) in gastric cancer (GC) tissues and GC cell lines, and the role of PTBP1 in the proliferation and metastasis of GC cells.Methods:From January to June in 2019 at The First Affiliated Hospital of Xi′an Jiaotong University, the cancer tissues and corresponding para-cancer tissues of GC patients underwent surgical resection were collected. The Kaplan-Meier Plotter database was used to analyze the survival of GC patients. The expression of PTBP1 was down-regulated by transfecting small interfering RNA (siRNA) in human GC cell lines SGC7901 and AGS with relatively high expression of PTBP1. The cells were divided into blank control group, negative control group, and PTBP1 knockdown group. The expression of PTBP1 at mRNA and protein level were detected by real-time fluorescence quantification polymerase chain reaction (RT-qPCR) and Western blotting. At 24, 48, 72 and 96-hour after transfection, the effect of PTBP1 on the proliferation of GC cells was observed by 3-(4, 5 dimethylthiazol)-2, 5 diphenyltetrazolium bromide (MTT) experiment. The changes of invasion and migration of GC cells after down-regulation of PTBP1 were detected by transwell assay. The expression changes of epithelial-mesenchymal transition (EMT) markers E-cadherin, N-cadherin and vimentin after down-regulation of PTBP1 in GC cells were determined by Western blotting. Indenpendent samples t test, analysis of variance and rank sum test were used for statistical analysis. Results:The Kaplan-Meier Plotter prognostic analysis showed that the overall survival of GC patients with high PTBP1 expression was shorter than that of GC patients with low PTBP1 expression (9.2 months, 6.2 months to 17.2 months vs. 19.0 months, 14.5 months to 28.4 months), and the difference was statistically significant ( Z=5.31, P<0.05). The results of RT-qPCR showed that in GC cell lines SGC7901 and AGS, the expression of PTBP1 at mRNA level of PTBP1 knockdown group was lower than that of blank control group and negative control group (SGC7901: 0.78±0.11 vs.3.10±0.19 and 2.99±0.23; AGS: 0.80±0.09 vs. 3.55±0.24 and 3.50±0.18), and the differences were statistically significant ( tSGC7901=10.57 and 8.08, tAGS=10.91 and 13.42; all P<0.01). The results of Western blotting indicated that in GC cell lines SGC7901 and AGS, the expression of PTBP1 at protein level of PTBP1 knockdown group was lower than those of blank control group and negative control group (SGC7901: 0.38±0.04 vs. 1.42±0.05 and 1.35±0.09; AGS: 0.17±0.02 vs. 1.52±0.08 and 1.38±0.45), and the differences were statistically significant ( tSGC7901=15.94 and 10.57, tAGS=16.60 and 20.80; all P<0.01). The results of MTT showed that at 48, 72 and 96-hour after transfection the absorbance values of PTBP1 knockdown group decreased by 0.25±0.01, 0.38±0.02, and 0.84±0.04 as compared with those of negative control group, and the decrease was the most significant at 96-hour after transfection, and the differences were statistically significant ( t=10.21、14.32, both P<0.01). The results of transwell experiment demonstrated that the number of invasion and migration cells of PTBP1 knockdown group were both less than that of the blank control group and the negative control group (SGC7901: 42.00±5.91 vs. 116.40±10.23 and 114.40±10.43; 39.60±6.77 vs. 125.80±11.51 and 122.40±5.90; AGS: 40.20±7.25 vs. 115.60±14.63 and 117.40±9.12; 36.00±5.20 vs. 122.40±12.10 and 125.40±12.74), and the differences were statistically significant ( tSGC7901=14.07, 13.50, 14.43 and 20.62; tAGS=10.27, 14.75, 14.68 and 16.76; all P<0.01). The results of Western blotting showed that the expression of E-cadherin of PTBP1 knockdown group was higher than that of the blank control group and the negative control group (SGC7901: 1.42±0.05 vs. 0.53±0.05 and 0.57±0.03; AGS: 1.34±0.04 vs. 0.54±0.03 and 0.61±0.01), however the expression levels of N-cadherin and vimentin were both lower than those of the blank control group and the negative control group (SGC7901: 0.50±0.03 vs. 1.64±0.05 and 1.46±0.07; 0.32±0.07 vs. 1.42±0.07 and 1.33±0.07; AGS: 0.37±0.06 vs. 1.47±0.04 and 1.36±0.04; 0.41±0.04 vs. 1.53±0.06 and 1.37±0.04), and the differences were statistically significant ( tSGC7901=11.63, 13.19, 18.83, 11.68, 11.43 and 10.43; tAGS= 15.02, 16.23, 14.67, 12.97, 14.45 and 17.18; all P<0.01). Conclusions:The expression levels of PTBP1 increase in GC tissues and cells, which may be involved in regulating the proliferation, metastasis and EMT of GC cells.

ABSTRACT

OBJECTIVE@#To delineate the blood group for a pair of twins with inconclusive ABO blood typing result.@*METHODS@#Serological test for blood group was carried out by using ABO and Rh Blood Grouping Cards (Microcolumn Gel Immunoassay). Sequence specific primer-PCR (PCR-SSP), direct sequencing and TA clone sequencing were used to analyze the ABO gene. Genetic status was analyzed by using 16 short tandem repeat (STR) markers.@*RESULTS@#Red blood cells of the twins displayed 2+ mixed agglutination phenomenon with anti-A, anti-A1 and anti-E. PCR-SSP and DNA sequencing of exons 6 to 7 revealed that they have an ABO*O.01.01/ABO*O.01.02 genotype. DNA sequencing of microsatellite enhancer region revealed presence of A gene. STR analysis revealed more than two haplotypes for 9 loci between the twins. After clustered by anti-A, the red blood cells were divided into two groups: A, CcDEe and O, CcDee, respectively.@*CONCLUSION@#Serological and molecular techniques have characterized the twins as blood group chimeras.

ABSTRACT

Objective@#To evaluate the feasibility and safety of magnetic tracer technique for preoperative endoscopic marking in laparoscopic surgery.@*Methods@#In the preliminary study, a total of 8 patients with gastric (n=3) or colorectal (n=5) tumors underwent endoscopic magnetic marking before laparoscopic surgery from April to June in 2019. First, a magnet was attached to the lesion by 2 titanium clips under the endoscope. Second, during the subsequent laparoscopic operations, the other magnet was sent to the vicinity of the lesion through the laparoscopic tunnel. The magnet in the abdominal cavity was quickly attracted to the one in the gastrointestinal tract to successfully locate the lesions. Data of preoperative marking and operations of 8 patients were reviewed.@*Results@#All 8 lesions were marked successfully, rapid and accurate intraoperative positioning was achieved. The mean time of endoscopic marking was 5.75±2.45 minutes, and the mean time of intraoperative localization was 1.94±0.56 minutes. All patients underwent laparoscopic tumor resections with accurate localization. The mean proximal and distal resection margins of colorectal tumors were 105 mm and 74 mm respectively. No complications occurred.@*Conclusion@#Magnetic tracer technique for laparoscopic localization, simple, safe and accurate for gastrointestinal lesions, can be performed without additional equipment or endoscopic procedures involved.

ABSTRACT

The healthcare-associated infections (HAI) cases monitoring system, automatic access to the medical record homepage and HAI early warning application of untreated early warning information interception and other means of information are introduced into the hospital. These means ensure the accuracy and integrity of the medical record homepage diagnostic and statistical data base information, and HAI consistency as well. Closed loop management of HAI cases provides accurate information for the hospital business intelligence platform, serving hospital administration and clinical management with accurate data. Hence precise decision-making basis can be provided for the improvement of medical quality of the hospital.

ABSTRACT

Objective To quantitatively compare the diagnostic capability of 68Ga-NGR and 18F-FDG in well-differentiated hepatocellular carcinoma (HCC) bearing mice by microPET/CT imaging.Methods The in vitro cellular uptake, in vivo microPET/CT imaging and biodistribution studies of 68Ga-NGR and 18F-FDG were quantitatively compared in SMMC-7721-based well-differentiated HCC.The human fibrosarcoma (HT-1080) and human colorectal adenocarcinoma (HT-29) cells/xenografts were respectively used as positive and negative reference groups for CD13.The expression of CD13 was qualitatively verified by immunohistostaining.The levels of CD13 and glucose-6-phosphatase (G6Pase) were semi-quantitatively analyzed by Western blot test for all 3 types of tumors.Two-sample t test was used for data analysis.Results The in vitro cellular uptake showed that the 68Ga-NGR uptake in SMMC-7721 and HT-1080 cells was higher than that in HT-29 cells, and the 68Ga-NGR uptake was higher than 18F-FDG uptake in SMMC-7721 cells.The in vivo microPET/CT imaging results revealed that the uptake of 68Ga-NGR in SMMC-7721 tumor was (2.17±0.21) %ID/g, remarkably higher compared to (0.73±0.26) %ID/g of 18F-FDG uptake (t=8.826, P<0.01).The tumor/liver ratio of 68Ga-NGR was 2.05±0.16, which was 2.03-fold higher than that of 18F-FDG.In the HT-1080 tumors, the uptakes of 68Ga-NGR and 18F-FDG were both high, and the values were (2.46±0.23) %ID/g, (3.47±0.31) %ID/g.The uptake of 68Ga-NGR was significantly lower than that of 18F-FDG in HT-29 tumors: (0.67±0.20) %ID/g vs (3.17±0.29) %ID/g;t=4.221, P<0.01.Western blot and immunohistostaining results were as follows: HT-1080(CD13+, G6Pase-), SMMC-7721(CD13+, G6Pase+), HT-29(CD13-, G6Pase-).Conclusions The uptake of 68Ga-NGR is higher than 18F-FDG uptake in SMMC-7721 tumor bearing mice, therefore it is worthwhile to consider the feasibility of clinical translation for PET/CT in diagnosis of HCC.Furthermore, because of the difference in 68Ga-NGR and 18F-FDG avidities in tumors with different molecular phenotypes of CD13 and G6Pase, there is an underlying potential for molecular imaging in the determination of molecular phenotypes.

Routine supervision on hospital infection control was normally fed back on paper-based media,plagued by low efficiency and poor traceability.A feedback process developed with Workflow technology,can loop the entire process ranging from routine supervision to identifying loopholes,to hospital infection and joint development of correctional measures clinically,all the way to implementation of infection control practice and sustained improvement.The closed-loop PDCA cycle management features greater efficiency and traceability in hospital management of its infection control,and builds a quality improvement model featuring close communication between infection control department and clinical departments.

ABSTRACT

Objective To investigate the characteristics of healthcare-associated infection(HAI) in elderly hospitalized patients,and analyze the risk factors for HAL Methods Incidences of HAI in elderly(elderly group) and non-elderly hospitalized patients (control) in a tertiary first-class hospital were investigated,the underlying diseases,infection sites,department distribution and so on between two groups of patients were compared.Results The incidence of HAI in elderly group was higher than that in control group(3.38 % vs 1.45 %,P<0.05);the proportions of hypertension and diabetes mellitus in elderly group was higher than control group,while cerebrovascular disease was lower than control group.The proportion of lower respiratory tract infection,urinary system infection and antimicrobial-associated diarrheal in the elderly were higher than control group(41.62% vs 28.00%,15.44% vs 12.02%,6.03% vs 3.15%,respectively).The incidence of catheter-associated urinary tract infection in the elderly group was higher than that in control group(6.85‰ vs 3.95‰,RR95%CI =1.209-2.485,P<0.05).The proportion of HAI in the elderly group in internal medicine departments was higher than that in control group (52.71% vs 40.03%),while in intensive care unit was lower than control group (33.52% vs 41.19%).Conclusion The characteristics of HAI in elderly patients are closely related to their demographic characteristics,early prevention of HAI in key sites should be carried out according to its characteristics.

ABSTRACT

Objective To improve the accuracy of medical students to put on and off the protective equipment with mind map joint microteaching approach teaching method. Method 90 medical students in clinical practice were randomly selected for teaching objects and divided into two groups, each group having 45 people. The control group used the traditional teaching while the experimental group was trained by mind map joint microteaching approach teaching method. And then, the examination table was conducted to each group and the assessment accuracy of the skills assessment results and excellent rates of the two groups were compared. Statistical analysis was performed by χ2 test. Results The average score of students of the experiment group in the whole set of putting on and off protective equipment and skills points assessment were higher than the control group, and the difference was statistically significant [(91.40±6.61) vs. (80.09± 6.28), P=0.001; (12.13±2.26) vs. (8.78±2.45), P=0.001]. The excellent rate of students of the experiment group in putting on and off protective equipment assessment and skills points examination was higher than the control group, and the difference was statistically significant (80%vs. 35%, P=0.001;71%vs. 13%, P=0.001). Conclusion Using innovative mind map joint microteaching approach teaching method can im-prove the medical students' operating skills and excellent rate s in putting on and off the protective equip-ment, and at the same time enhance their learning interest and professional protection level.

ABSTRACT

Objective On the background of the clinical skills competition for national medi-cal students in the higher medical colleges and universities, to investigate medical students' mastery of hand hygiene knowledge and skills, explore the effective methods of teaching, and improve the medical students' hand hygiene compliance and accuracy in clinical practice. Methods 90 medical students at a tertiary-level teaching hospital clinical practice stage were randomly selected for the study, and performed hand hygiene theoretical knowledge and practical skills training in special education. Special teaching investigation was made before and after training, to investigate the students' hand hygiene compliance rate in clinical practice. And the correct rate of hand hygiene were examined and medical students' hand hygiene compliance rate and correct rate were compared before and after the special teaching and training. Statistical analysis was made by using chi square test 2. Results After theory teaching and additional special skills training the students' hand hygiene compliance and accuracy were improved significantly (79.00%vs. 35.50%, P=0.000). The correct rate of hand hygiene was also significantly higher than before training (92.22% vs. 61.11%, P=0.000). Conclusion Hand hygiene knowledge and skills training can significantly improve the medical students' hand hygiene compliance and accuracy in the clinical practice, and is conducive to the prevention of the spread of infection in hospitals, and then reduce the incidence of hospital infection.

ABSTRACT

In accordance with the General Hospital Accreditation Standards Ⅲ (2011) and other HAI-related hospital guidelines,a quantitative assessment index system for infection control of general hospitals has been established.Based on these,departmental performance of the hospital has been regularly checked in their infection control,and the outcomes of assessment included into the performance appraisal system of the hospital.Thanks to this practice,there have been no outbreaks of hospital associated infections in the hospital in recent two years,the incidence of hospital associated infection decreased from 1.25% in 2012 to 1.05% in 2013.Proper hand hygiene rates,compliance rates increased from 45.3% and 26.9 % in 2012 to 92.4 %and 65.5 % in 2013.The index system for evaluating prevention and control of hospital associated infection is scientific and effective.

ABSTRACT

BACKGROUND: The femoral neck fracture is a common skeletal injury, with high mortality and disability rate. Nonunion and avascular necrosis in femoral head are the main problems in treatment of femoral neck fracture. Up to now, the guidelines for its management continue to be unsolved and are stil evolving. OBJECTIVE:This study aims to summarize the incidence of two femoral neck fracture complications, as wel as the advance in prevention and treatment. This work can help to better understand the mechanism of non-union and avascular necrosis, and to choose the optimal management of this fracture. METHODS:Papers in terms of femoral neck fracture published in core periodicals were reviewed. Studies which are not directly related to femoral neck fracture or articles regarding poly-trauma were excluded. Literatures concerning non-union and avascular necrosis after femoral neck fracture were preferred. RESULTS AND CONCLUSION:A total of 241 articles were screened out, and those were mainly published in the last five to ten years. 182 unrelated papers were excluded, and 59 papers were included in the analysis. The femoral neck fracture has a high incidence. Nowadays, there are a variety of treatments for this fracture, but the stability and healing result of fractures depend on many factors, such as age, classification of fracture, general condition of patient, and accompanying diseases. Up to now, the choice of management is stil controversial. Imaging representation and patient’s age are not the main factors for the selection of treatment. Hence, a comprehensive assessment of the risk factors that influence fracture healing is necessary for surgeons to choose an optimal treatment of this fracture. Femoral neck fractures, therefore, represent an enormous socio-economic and medical problem and chal enge surgeons to find the cheapest and most effective way to treat them. Internal fixation of the fracture in combination with the use of osteo-inductive materials may be a new way to further improve the treatment effects of femoral neck fracture.

BACKGROUND:As a lightweight metal with mechanical properties similar to natural bone, a natural ionic presence with significant functional roles in biological systems, and in vivo degradation via corrosion in the electrolytic environment of the body, magnesium (Mg)-based implants have the potential to serve as biocompatible, osteoconductive, degradable implants for load-bearing applications. On the other hand, Mg, as a critical mediator of cellular growth and activities regulation, is the second most abundant intracellular cation where it plays an important role in enzyme function and trans-membrane ion transport. OBJECTIVE:To retrospectively summarize the application progress, physiological role and mechanism of Mg, and to review its regulation of bone metabolism. METHODS:Papers addressing application, physiological and bone metabolism regulation of Mg published in core periodicals were reviewed. Studies in which Mg were used as an assistance materials, or articles regarding cellphysiology in which Mg were not the main research object were excluded. Literatures concerning the regulation of bone turnover by Mg were given special attention. RESULTS AND CONCLUSION:Mg al oys are cytocompatible materials with adjustable mechanical and corrosion properties and show their potential as biodegradable implant materials. Moreover, Mg is abundantly distributed among the body and essential for maintaining physiological function of the cells. Mg deficiency has been associated with a number of clinical disorders including osteoporosis. But it remains to be determined what specific biochemical process is activated by Mg2+to regulate cellular activity and bone turnover. Further investigations of the precise mechanism are valuable to the clinical application of Mg.

ABSTRACT

Objective To explore the investigation and control method for the epidemic of surgical site infection (SSI)in a hospital.Methods Data of orthopedics patients receiving clean wound orthopedics operation were ana-lyzed retrospectively,and effective measures were taken to control the epidemic of HAI.Results From June to August 2012,a total of 312 patients received clean wound orthopedics operation,7 (2.24%)SSI cases occurred.SSI rates during the same period from 2009 to 2012 were significantly different(P <0.05),and was the highest in 2012. HAI management staff reviewed patients’medical records,communicated with staff in orthopedics department,and put forward advice for SSI control;orthopedics department worked out and implemented detailed infection control measures.The subsequent surveillance showed that the prevalence of SSI in orthopedics ward decreased to 0.38%, infection outbreak was effectively controlled.Conclusion HAI management department played an important role in the finding and control of HAI outbreak.Scientific analysis on monitored data and proper measures adopted by HAI management department and clinical department can effectively control HAI outbreak.

BACKGROUND:Alternative implants affect stability of prostheses,and freeze-dried bone allografts are most selected as implants.The crack between grafts and host bone is unavoidable,which would weaken the bone integration.Bone cement can fill the cracks quickly and completely,however,the effects of bone cement on the stress of prostheses and its own remains poorly understood.OBJECTIVE:To evaluate the effect of the different grafting for bone loss in the presence of the new acetabular component with wings at the time of acetabular revision.METHODS:The finite-element models of acetabular component with three wings and acetabulum with bone loss were established.Following prostheses implantation,the gaps were filled with bone cement and freeze-dried bone allograft,respectively,stress of the graft and its effect on prostheses were analyzed under 2 158 N and 426 N acetabular loads.RESULTS AND CONCLUSION:The finite-element analysis demonstrated that stress and strain at the interfaces of bone-shell and metal-polyethylene liner had the same direction of change but no significant difference between them.In the study,the result indicated increased contact stresses with an increased hip force and orientation.The stress of wings increased gradually from rim to root.Its peak stress was at the joint between the wing and the shell.The different grafts did not affect the stress of the acetabular component.But the stress of bone cement was significantly higher than that of particulate bone.The study demonstrated that change of the graft could not make remarkable effect on the stress of the acetabular component.But the stress of bone cement increased significantly.The result of the finite-element analysis indicated that particulate bone graft is benefit to improving itsstability and to minimize aseptic loosening rate of acetabular component.

ABSTRACT

BACKGROUND:The main factors that influence revision of acetabular bone defects include evaluation of acetabular bone defects,reconstruction of acetabulum and appropriate acetabular cups used in revision.OBJECTIVE:The review from the three aspects will help to choose an appropriate cup in revision and to formulate the best revision protocols.METHODS:A computer-based online search of PubMed was performed for English articles published from January 1900 to June 2009 with the key words of acetabulum,revision,bone defect,in English and Chinese.Clinical studies published in core periodicals of the latest twenty years were reviewed.Articles regarding primary joint replacement,femoral prosthesis revision and animal experiments were excluded.RESULTS AND CONCLUSION:A total of 61 articles were analyzed,31 were excluded,and 30 mainly published in 5 years,were included.Reconstruction of acetabular bone structure,restoration of original rotation center of hip joint are key factors for repair.Moreover,appropriate prosthesis further benefits the repair effect.Bone transplantation for bone defect and cementedless prosthesis have become favorable methods for revision of acetabular bone defects.

ABSTRACT

Objective To analyze and evaluate a new acetabular revision component with three wings. Methods The finite-element models of a new acetabular component and acetabulum with bone loss were established to calculate the interface stresses during a normal gait cycle with use of the finite-element formulations. Results The finite-element analysis demonstrated that stress and strain at the interfaces of bone-shell and metal-polyethylene liner had the same direction of change but no remarkable difference between them. In the study, the result indicated increased contact stresses with an increased joint load and orientation. The peak stress was tested at the second stage of gait cycle. The stress of wings increased gradually from rim to root. Its peak stress that was significantly lower than yield force of the Co-Cr alloy was at the joint between the wing and the shell. The stress of graft had the same change rule as the joint force. The part of graft near to acetabular component was subjected to higher stress conditions. Conclusion The hip forces can transfer from acetabular component and implant to acetabulum. The result of the finite-element analysis underlined the importance of wings of the new acetabular component. The wing can help to improve the antitorsion ability of acetabular component and to minimize its aseptic loosening rate. Therefore, based on results of this study and clinical application, the acetabular component with wings is known a viable means for acetabular revision in the presence of bone loss. But further research is needed as to this acetabular component.

ABSTRACT

Alternative designs have been explored in an attempt to improve the longevity of acetabular prostheses in revision surgery. Many studies had been designed to test the result of the extra-large uncemented hemispherical acetabular components used for acetabular revision in the presence of bone loss. A clinical study of a new acetabular component consisting of a porous metal shell with three wings and an all-polyethylene liner had attained a satisfying result. The purpose of this study was to evaluate the new acetabular component by comparison with the hemispherical acetabular component. The finite-element models of the two acetabular components and the acetabulum in the presence of bone loss were established to calculate the interface stresses during a normal gait cycle with use of the finite-element formulations. Results of the finite-element analysis demonstrated that stress and strain of the two acetabular components had the same direction of change, but the new component had higher stress at the root of wings. In the study, the result indicated reduced contact stresses with a reduced abduction angle of wings. The stress was lower in the new acetabular component of 15° angle of wings compared with the new acetabularcomponent of 30°of wings and hemispherical acetabular component (P < 0.05). The stress of acetabular component with wings showed no remarkable difference by comparison with the extra-large uncemented hemispherical acetabular component. Reducing abduction angle of wings seemed to be a viable means of reducing the stress of the acetabular component while improving its stability.

ABSTRACT

OBJECTIVE To study relativity of systemic inflammatory response syndrome(SIRS) score and nosocomial infection(NI) and outcome in ICU patients.METHODS A total of 1897 ICU patients were enrolled in our study.The SIRS within 48 hours after admission was recorded.And the prospective evaluation in NI and prognosis was made.RESULTS The nosocomial infection rate was 10.1%,and the mortality was 9.9%.With SIRS score increasing,NI and mortality rates increased as well and the tendency presented a linear correlation.CONCLUSIONS As a simple system,SIRS score can initially predict the risk of NI and outcome in ICU patients.

ABSTRACT

OBJECTIVE To study the risk factors of hospital acquired urinary system infection in neurology ICU(NICU) and emergency ICU(EICU).METHODS The general character,score of APACHE Ⅱ and the time length of urethral catheterization of the 502 NICU and EICU patients were enrolled in our study.Then the relativity of the risk factors and hospital acquired urinary system infection were investigated.RESULTS Urethral catheterization increased the chances of hospital acquired urinary system infection and there were more chances with the long time.CONCLUSIONS In order to decrease the rate of hospital acquired urinary system infection we must strictly grasp the indication of urethral catheterization and pull out the urethral catheter as early as possible.
